User Authorization Form
Use this form to give permission for a specific person (“Authorized User”) to access and submit information in the Office of the State Auditor’s Form Entry System (SAFES) and certain offline documents. This form is required if the individual is performing work on behalf of a local government, TIF authority, or fire relief association (“Entity”) and is not an officer, employee or trustee of the Entity.
Access will remain valid through December 31 of the User Authorization Form year, unless an earlier end date is entered on the form. Access can be canceled at any time by sending a written request to the Office of the State Auditor (OSA). After the form has been completed, printed, and signed, it can be submitted by email, fax, or US Mail to the appropriate OSA division(s).
